In addition to the insurance services above, we are affiliated with several other networks, smaller insurance plans, and replacement plans. If your insurance is not listed here, please call us and ask to speak to one of our insurance specialists to see if we have a relationship with your provider.

In order for us to accept your insurance, you will be expected to present a picture ID and your insurance card(s) at your initial visit with us. Estimated payments are due at the time of service. When appropriate, co-pays are also collected prior to the visits. In some cases, tests and procedures require pre-authorization from insurance companies. We are committed to delivering exceptional care to every patient, and are prepared to work with you to obtain the needed authorizations in a timely manner.

In order to provide accessible treatment to our patients, we also accept payment in the form of cash, checks and credit cards. If you have any questions concerning the cost of treatment or insurance , please don’t hesitate to give us a call.
